What do I need to bring to my appointment?

Please bring your photo ID, as well as your Medicare Part B (RED, WHITE and BLUE card) or private medical and prescription insurance cards.

What is the cost of the vaccines?

Most insurances cover routine vaccinations with a $0 copay. We will let you know if there is any out-of-pocket costs with your specific insurance prior to vaccination.

Are there forms to fill out?

Yes! If you have access to a printer, you can download and print the forms so you can fill out in advance.

What if I can’t print the form?

Please arrive 5-10 minutes early and go through our drive thru to receive the necessary form prior to your appointment time.

How early should I arrive?

Your appointment guarantees you will receive a vaccine so there is no need to arrive early! Arriving no more than 5 minutes early allows us to ensure social distancing.

What should I wear?

Remember to wear short sleeves or loose clothing for easy access to your upper arm. Masks covering your nose and mouth are required during vaccination.

What should I expect after receiving a vaccination?

  • After your vaccination, you will need to wait at least 15 minutes before leaving the clinic site so you can be observed for any allergic reactions.

  • The most common side effects of vaccines include: pain and swelling at the injection site, swollen lymph nodes, low grade fever, chills, headache, fatigue or muscle aches.
