What do I need to bring to my appointment?
Please bring your photo ID, as well as your Medicare Part B (RED, WHITE and BLUE card) and any private medical and prescription insurance cards.
Is there a cost for the COVID-19 vaccine?
There is no charge to you for the vaccine or the administration fee. However, we will bill your insurance, if applicable, for the administration fee. Please note that Medicare covers the administration fee for all Medicare Advantage customers so it will be necessary to bring your Medicare red, white and blue card.
Are there forms to fill out?
Yes! If you have access to a printer, you can download and print the forms so you can fill out in advance.
For those 12 years of age and older, please use the following form: D&H Vaccine Screening and Consent Form.
For those 5-11 years old, please use the following form: D&H Pediatric Vaccine Screening and Consent Form.
What if I can’t print the form?
Please arrive 5-10 minutes early and go through our drive thru to receive the necessary form prior to your appointment time.
How early should I arrive?
Your appointment guarantees you will receive a vaccine so there is no need to arrive early! Arriving no more than 5 minutes early allows us to ensure social distancing.
What should I wear?
Remember to wear short sleeves or loose clothing for easy access to your upper arm.
Please wear a mask over your nose and mouth.
Who should not get the COVID-19 vaccine?
Anyone who had a severe allergic reaction (e.g., anaphylaxis) after a previous dose of an mRNA COVID-19 vaccine or any of its components
Anyone who had an immediate allergic reaction of any severity to a previous dose of an mRNA COVID-19 vaccine or any of its components (including polyethylene glycol [PEG])*
Anyone who had an immediate allergic reaction of any severity to polysorbate (due to potential cross-reactive hypersensitivity with the vaccine ingredient PEG)*
Anyone currently under quarantine for exposure to COVID-19 or isolating due to a positive COVID-19 test.
Please note: If you have had a severe allergic reaction (e.g., anaphylaxis) to an mRNA COVID-19 vaccine and wish to receive the Johnson & Johnson vaccine, you will need to make an appointment with a facility that has advanced medical care available. D&H Drug Store is not considered an advance medical care facility.
What should I expect after receiving the COVID -19 vaccination?
After your vaccination, you will need to wait at least 15 minutes before leaving the clinic site so you can be observed for any allergic reactions.
The most common side effects include: pain and swelling at the injection site, swollen lymph nodes, low grade fever, chills, headache, fatigue or muscle aches.
